Weekends: each physician works every 2nd or 3rd weekend. Avoid back-to-back weekends unless their time-off or swap requests dictate otherwise.
Circadian progression: keep a physician on the same shift for 1–2 weeks before advancing to the next later start time. Swapping in a shift with a start time within a few hours is acceptable when needed for fairness.
Fairness: prioritize an even distribution of night shifts and weekend shifts across all physicians whenever possible.
Provide at least 48 hours off after a run of night shifts before scheduling a day shift.
No physician works more than 3 night shifts in a row.
No physician works more than 5 consecutive shifts of any type.
Always ensure every shift is fully covered by a qualified physician.
Spread any unfilled shifts evenly throughout the month and concentrate them on weekday day shifts rather than weekends or nights whenever possible.
